Arrangements at Jordan-Fernald

When making funeral arrangements, it is helpful to bring these items to the meeting:
  • Birth certificate
  • List of surviving relatives and their city and state of residence
  • A list of personal information such as membership in organizations or hobbies of the deceased
  • Cemetery deed
  • Military discharge papers (Form DD214)
  • Recent photograph for obituary and/or for cosmetizing and hair style
  • Clothing, including under garments and outer clothing of your choice. An outfit with a closed neckline and long sleeves is preferable.
  • Personal items such as eye glasses and any religious items.
You can get a head start on the arrangement process by completing as much of the necessary details as possible in the comfort of your own home. We recognize you may not know everything right at this moment, but what you do know will be invaluable to your Funeral Director and downloading and completing this Arrangement Worksheet will expedite the funeral arrangement process. If you are looking to pre-arrange your services, our online pre-planning tool can walk you through the process.
